Fill Out an Application To apply for a CODA project instrument, applicants must meet the following requirements:

1. The student is officially enrolled in a participating school band or orchestra program.

2. The student has demonstrated good work ethic and behavior in the classroom.

3. The student has agreed to show respect for the instrument and will care for it while in their possession.

4. The student qualifies for free or reduced lunch program at school, or is being nominated by a music teacher.

5. The student’s parent(s) or guardian(s) have been unable to obtain an instrument through conventional means.

The application needs to be filled out and signed by all 3 parties, the parent/guardian of the student, the music teacher, and the principal or administrator for the school. All applications need to be completely filled out in order to submit. Any application that is not filled out in entirety will be sent back for correction and could delay the process.

2. Upload and submit your application here.

3. E-mail a copy of your application to thecodaproject@gmail.com

4. Mail your application to: Attn: CODA Project 5976 Executive Dr. Ste. A Fitchburg, WI 53719

5. Submit your application through your teacher via a Ward-Brodt Representative that calls on your participating school.

All applications will remain confidential and your information will not be shared outside of our organization or participating partners.

Review.

Once an application is received it will enter the review stage.

1. We will notify you within 24 hours to let you know we have received your application.

2. We will then review the application to ensure it has been filled out completely and signed by all parties.

3. We will then determine if the student qualifies for a CODA instrument.

4. We will check availability to ensure we have an instrument based on the student's selection choices. If we currently do not have an instrument for that student, we will reach out to give you your options. You may either select another instrument or be placed on a waiting list for the instrument you desire.

5. Once the review stage is complete we will fill your instrument request and an e-mail confirmation will be sent out.

6. We will then work with our team to ensure delivery of your CODA rental instrument.

The average turn around time for a CODA application is 7-10 Days.
